Using and Saving the PDF Form
To open the form in your browser, click on this
Closing Request Form
link. To save the form, you must use the save icon just above the form. The
browser "File > Save" option does not apply to PDFs displayed in the
browser.
To save the form to your
computer, right-click on the application form link and select "Save
Target As..."
Attention: You may not be able to save the PDF form after you have entered data, or you may lose the data when you do save. The free version of Adobe Acrobat (Acrobat Reader) does not allow users to save a modified PDF file.
Note: If you do not have the free Acrobat Reader, visit the Adobe website at http://www.adobe.com to download it. We recommend the latest version 7.0 for best performance.
